本项目第一标段为专门面向中小企业项目，供应商应为中型企业、小型企业、微型企业或监狱企业或残疾人福利性单位。供应商为中型企业、小型企业、微型企业的，提供《中小企业声明函》，且中小企业的划分标准所属行业为物业管理；供应商为监狱企业的，应提供监狱企业的证明文件；供应商为残疾人福利性单位的，应提供《残疾人福利性单位声明函》。













3.本项目的特定资格要求：





合同包1(陕西中医药大学附属医院工作区物业服务)特定资格要求如下:





（1）营业执照等主体资格证明文件：提供有效存续的企业营业执照（副本）/事业单位法人证书/专业服务机构执业许可证/民办非企业单位登记证书。

？？？？（2）财务状况报告：提供2020年度或2021年度经审计的财务会计报告（包括审计报告、资产负债表、利润表、现金流量表、所有者权益变动表及其附注，成立时间至提交投标文件截止时间不足一年的可提供成立后任意时段的资产负债表），或其开标前三个月内基本存款账户开户银行出具的资信证明，或信用担保机构出具的投标担保函。（以上三种形式的资料提供任何一种即可）。

？？？？（3）社保缴纳证明：提供递交投标文件截止之日前一年内任意一个月的社会保障资金缴存单据或社保机构开具的社会保险参保缴费情况证明。依法不需要缴纳社会保障资金的投标人应提供相关证明文件。

1.供应商投标须知



（1）本项目采用电子化投标及远程不见面开标方式。供应商须使用数字认证证书（CA锁）对电子投标文件进行签章、加密、递交及开标时解密等相关招投标事宜。开标前，供应商需登录网络开标大厅。开标时，按照工作人员要求进行远程解密，如因供应商自身原因造成无法解密投标文件，按无效投标对待。



（2）制作电子投标文件。投标人须在“全国公共资源交易中心平台（陕西省）（
