（1）、法定代表人或负责人授权书（附法定代表人或负责人身份证复印件）及被授权人身份证；（法定代表人或负责人直接参加投标只须提供法定代表人或负责人身份证）

（2）、供应商为制造商的，须具备特种设备制造许可证锅炉A级、锅炉安装（含维修、改造）许可证二级或B级或以上资质；供应商为代理商的，须具有锅炉生产制造商特种设备制造许可证锅炉A级复印件加盖制造商公章（鲜章）、代理商还须具备特种设备锅炉安装（含维修、改造）许可证二级或B级或以上资质。

（3）、不得为“信用中国”网站(

（4）、单位负责人为同一人或者存在直接控股、管理关系的不同投标人，不得参加同一合同项下的政府采购活动。
