3.本项目的特定资格要求：





合同包1(子长市人民医院多功能影像平台系统采购)特定资格要求如下:





（1）、投标人应在中华人民共和国境内注册的企业法人、事业法人、其他组织或自然人，出具合法有效的营业执照或事业单位法人证书，自然人参与的提供其身份证明；（2）、投标人为制造厂家的应出具医疗器械生产许可证（提供的设备须在其生产范围内）和所投设备医疗器械注册证；投标人为经销商的应出具医疗器械经营许可证（提供的设备须在其经营范围内）和所投设备的医疗器械注册证；（3）、税收缴纳证明：提供响应文件递交截止日前一年内已缴纳的至少一个月的纳税证明或完税证明，依法免税的单位应提供相关证明材料；（4）、社会保障资金缴纳证明：提供响应文件递交截止日前一年内已缴存的至少一个月的社会保障资金缴存单据或社保机构开具的社会保险参保缴费情况证明，依法不需要缴纳社会保障资金的单位应提供相关证明材料；（5）、投标人具有履行合同所必需的设备和专业技术能力的承诺函；（6）、投标人出具参加政府招标活动前3年内在经营活动中没有重大违法纪录的书面声明；（7）、单位负责人为同一人或者存在控股、管理关系的不同投标人，不得同时参加本项目采购活动；（8）、投标人未被列入“信用中国”网站严重失信主体名单和税收违法黑名单；政府采购严重违法失信行为记录名单中被财政部门禁止参加政府采购活动的投标人；（9）、法定代表人直接参加投标只须提供法定代表人身份证，非法定代表人投标，须出具法定代表人授权书及被授权人身份证；
