3.本项目的特定资格要求：





合同包1(洛南县中医医院迁建项目(监理))特定资格要求如下:





(1)提供合法有效的营业执照或者其他组织的证明文件；

(2)提供法定代表人授权书（附法定代表人、被授权人身份证复印件）及被授权人身份证原件（法定代表人直接参加投标的，须提供法定代表人身份证原件及复印件）；

(3)不得为“信用中国”网站（

(4)参加政府采购活动前3年内，在经营活动中没有重大违法记录的书面声明及具有履行本合同所必需的设备和专业技术能力的承诺函；

(5)提供投标截止日前6个月内任一个月的纳税证明或完税证明(任意税种),依法免税的单位应提供相关证明材料；

(6)提供投标截止日前？6个月内任一个月的社会保障缴费凭据或社保机构开具的社会保障参保缴费情况证明；依法不需要缴纳社会保障资金的应提供相关文件证明；

(9)拟派项目总监理工程师具备房屋建筑工程注册监理工程师执业资格，且未担任其他在建工程项目的总监理工程师；

(10)信誉良好，供应商及拟派项目总监理工程师基本情况必须在《陕西省建筑市场监管与诚信信息发布平台》上可查询（附查询截图并加盖公章）
