3.本项目的特定资格要求：





合同包1(石泉县中医医院医养结合康复医院设计采购项目)特定资格要求如下:





（1）提供有效合格的社会统一信用代码的营业执照，其他组织经营的须提供合法凭证，自然人的提供身份证明文件;

（2）法定代表人授权委托书、被授权人身份证（法定代表人参加投标时,只需提供法定代表人身份证）；

（3）投标人须具备城乡规划编制乙级及以上资质；

（4）投标人具有建设行政主管部门颁发的工程设计综合甲级资质或建筑行业设计乙级及以上资质或建筑行业（建筑工程）专业设计乙级及以上资质；

（5）拟派设计负责人应具备一级注册建筑师及以上执业资格，且在本单位注册（提供社保证明）；

（6）财务状况报告：提供2021年度经审计的财务报告，或其基本存款账户开户银行出具的资信证明及基本存款账户开户许可证（基本账户信息表）；

（7）税收缴纳证明：提供2022年1月至今已缴纳的至少一个月的纳税证明或完税证明（任意税种），依法免税的单位应提供相关证明材料；

（8）社会保障资金缴纳证明：提供2022年1月至今已缴存的至少一个月的社会保障资金缴存单据或社保机构开具的社会保险参保缴费情况证明，依法不需要缴纳社会保障资金的单位应提供相关证明材料；

（9）具备履行合同所必须的设备和专业技术能力的书面声明;

（10）参加政府采购活动前三年内，在经营活动中没有重大违法记录的书面声明；

（11）投标人未被列入信用中国网站(
