3.本项目的特定资格要求：





合同包1(丹凤县2022年度供水保障提升项目)特定资格要求如下:





3.1、具有独立承担民事责任能力的法人或其他组织，提供合法有效的统一社会信用代码的营业执照；

3.2、法定代表人委托授权书及被授权人身份证，法定代表人参加磋商的只需提供法定代表人身份证；

3.3、供应商不得为“信用中国”网站（

3.4、财务状况：提供上年度年度的财务审计报告（至少包括资产负债表和利润表，成立时间至提交投标响应文件截止时间不足一年的可提供成立后任意时段的资产负债表），或其基本存款账户开户银行出具的资信证明及基本存款账户开户许可证；

3.5、税收缴纳证明：提供投标截止日前近一年内任意一个月的纳税证明或完税证明（任意税种），依法免税的单位应提供相关证明材料；

3.6、社会保障资金缴纳证明：提供投标截止日前近一年内任意一个月的社会保障资金缴存单据或社保机构开具的社会保险参保缴费情况证明，依法不需要缴纳社会保障资金的单位应提供相关证明材料；

3.7、参加政府采购活动前3年内，在经营活动中没有重大违法记录的书面声明；

3.8、省级以上卫生部门批准的涉及饮用水卫生安全产品的卫生许可批件；
