要求中小企业以联合体参加，且联合体中中小企业达到30.0%：

1、采购人预留该部分采购项目预算总额的30%以上专门面向中小企业采购，其中预留给小微企业的比例不低于60%。（a、如果代理商参加本项目投标，则货物须由中小企业制造商供货金额达到30%以上，且此部分份额由小微企业制造商供货金额达到60%以上。

b、如果制造商参加本项目投标，本身为小微企业可独立供货，如为中小型企业须与小微企业制造商组成联合体,小微企业供货比例不低于上述标准，如果为大型企业须与中小微企业组成联合体，供货比例不低于上述标准。

c、投标单位根据上述情形自行提供相应承诺书，格式自拟。）

3.本项目的特定资格要求：

(1)拟参与本项目的投标人所投产品如果为以下类别产品应具备相应品类之一的证件： 第一类医疗器械：提供所投产品的医疗器械生产企业备案凭证和医疗器械备案凭证； 第二类医疗器械：具有第二类医疗器械经营备案凭证（代理商作为供应商适用），并提供所投产品的医疗器械生产许可证和医疗器械注册证； 第三类医疗器械：具备医疗器械经营许可证（代理商作为供应商适用），并提供所投产品的医疗器械生产许可证和医疗器械注册证。供应商根据所投产品类别提供相应证明材料。
