3.本项目的特定资格要求：





合同包1(周至县教育和科学技术局公办园2022年校车服务公司项目)特定资格要求如下:





（1）营业执照等主体资格证明文件：提供有效存续的企业营业执照/事业单位法人证书/专业服务机构执业许可证/民办非企业单位登记证书。

（2）财务状况报告：提供2020年度或2021年度经审计的财务会计报告，成立时间至提交投标文件截止时间不足一年的可提供成立后任意时段的资产负债表），或其开标前三个月内基本存款账户开户银行出具的资信证明。

（3）社保缴纳证明：提供已缴存的2021年5月1日以来至少一个月的社会保障资金缴存单据或社保机构开具的社会保险参保缴费情况证明。依法不需要缴纳社会保障资金的投标人应提供相关文件证明。

（4）税收缴纳证明：提供已缴纳的2021年5月1日以来至少一个月的纳税证明或完税证明。依法免税的投标人应提供相关文件证明。

（5）信用记录：投标人未被列入“信用中国”网站记录的“失信被执行人”或“重大税收违法案件当事人”名单；不处于“”记录的“政府采购严重违法失信行为记录名单？”中的禁止参加政府采购活动期间。

（6）提供参加本次政府采购活动前三年内在经营活动中没有重大违法记录的书面声明；

（7）控股管理关系：提供直接控股和管理关系清单。若与其他投标人存在单位负责人为同一人或者存在直接控股、管理关系的，则投标无效。

（8）书面声明：提供书面声明，包括声明具有履行合同所必需的设备和专业技术能力；

（9）法定代表人授权委托书：法定代表人参加投标的，须提供本人身份证复印件（附在资格证明文件中）；法定代表人授权他人参加投标的，须提供法定代表人授权委托书。
