合同履行期限：自合同生效之日起30个日历天内完成交货并安装。

二、申请人的资格要求：

1.投标供应商应具备《政府采购法》第二十二条规定的条件，提供下列材料：

1）具有独立承担民事责任的能力：提供在中华人民共和国境内注册的法人或其他组织的营业执照复印件具有从事本项目的经营范围。（或事业单位法人证书，或社会团体法人登记证书）、组织机构代码证、税务登记证【如已办理了多证合一，则仅需提供合证后的营业执照复印件】）；

2）有依法缴纳税收和社会保障资金的良好记录：提供2021年1月至今＜至少提供任意一个月＞的缴纳税收的凭据证明材料复印件；如依法免税的，应提供相应文件证明其依法免税；提供2021年1月至今＜至少提供任意一个月＞的社会保险的凭据证明材料复印件；如因疫情影响当地政府部门允许缓缴社保的，应提供当地政府部门下发的相关文件作为证明材料，否则不予认可；如依法不需要缴纳社会保险的，应提供相应文件证明其依法不需要缴纳社会保障资金；

3）具有良好的商业信誉和健全的财务会计制度：提供2020年度财务状况报告关键页复印件＜至少含有资产负债表、利润表和现金流量表＞，或2021年1月至今＜至少提供任意一个月＞的财务状况报告复印件，或开户银行出具的资信证明材料复印件；

4）履行合同所必须的设备和专业技术能力：按投标文件格式提供《投标人资格声明函》；

5）参加采购活动前3年内，在经营活动中没有重大违法记录：出具声明书并加盖公章；重大违法记录，是指供应商因违法经营受到刑事处罚或者责令停产停业、吊销许可证或者执照、较大数额罚款等行政处罚。（较大数额罚款按照发出行政处罚决定书部门所在省级政府，或实行垂直领导的国务院有关行政主管部门制定的较大数额罚款标准，或罚款决定之前需要举行听证会的金额标准来认定）。

6）信用记录：投标人未被列入“信用中国”网站(www.creditchina.gov.cn)“记录失信被执行人或重大税收违法案件当事人名单或政府采购严重违法失信行为”记录名单；不处于(www.ccgp.gov.cn)“政府采购严重违法失信行为信息记录”中的禁止参加政府采购活动期间。（在“信用中国”网站（www.creditchina.gov.cn）及(www.ccgp.gov.cn)查询结果以投标人提供截图并加盖公章为准，如相关失信记录已失效，投标人需提供相关证明资料。查询有效期为自本项目采购公告发出之日起至递交投标文件截止时间止）；

7）供应商必须符合法律、行政法规规定的其他条件：单位负责人为同一人或者存在直接控股、管理关系的不同供应商，不得参加同一合同项下的政府采购活动（按投标文件格式提供《投标人资格声明函》）；为采购项目提供整体设计、规范编制或者项目管理、监理、检测等服务的供应商，不得再参加该采购项目的其他采购活动（按投标文件格式提供《投标人资格声明函》）；
