3、投标人资格要求

投标人应具备的资格或业绩：投标人必须是制造商或制造商授权的代理商，投标人如为代理商则必须提供制造商有效期内的授权书，否则其投标将被否决。1）关境内投标人须提供以下资质证书的复印件：①加载统一社会信用代码的营业执照或事业单位法人证书，且注册资金/开办资金不少于500万人民币或等值外币；②投标人需提供所投产品制造商在有效期内的ISO9001质量体系认证证书复印件或同等质量体系认证证书复印件；③重大涉诉情况说明（格式详见本章附件3加盖投标人公章）；④投标人需证明未被列入“信用中国”网站(www.creditchina.gov.cn)失信被执行人名单（需提供“信用中国”查询记录并加盖公章）。若投标人无法查询到其，则需提供相关截图。2）关境外投标人须提供以下资质证书的复印件：①有效的企业登记证明材料；②投标人需提供2020年度经审计的财务报告，且净资产不少于500万人民币或等值外币（财务报告中需体现净资产金额，并加以标注，否则将不予认定）；③投标人需提供所投产品制造商在有效期内的ISO9001质量体系认证证书复印件或同等质量体系认证证书复印件；④重大涉诉情况说明（格式详见本章附件3加盖投标人公章）；⑤投标人未成为失信被执行人承诺书（格式自拟）。业绩要求：近5年（2016年至今），投标人或所投产品制造商应具有制造和/或销售本招标要求的同规格或类似货物的不少于3份的业绩并提供业绩清单（清单中应列出销售时间、销售数量、货物名称、货物型号及最终用户等），同时提供近5年（2016年至今）投标人或所投产品制造商签署的不少于3份的销售合同或销售订单复印件作为证明材料。销售合同或订单的复印件须能体现签订时间，销售产品内容及数量，签订双方等关键信息，否则将被作为无效证明材料。银行资信证明：1）境内投标人应当提供在开标日前3个月内由其开立基本账户的银行开具的有效的银行资信证明；2）境外投标人应当提供在开标日前3个月内由其开户银行开具的有效的银行资信证明。
